Beverly M. Kozak

Beverly M. (Bev) Kozak 73, of Fort Atkinson, passed away peacefully on Thursday, June 23, 2011 surrounded by her family at Meriter Hospital in Madison.
Bev was born on May 1, 1938. She graduated from Cambridge High School in 1956. On April 11, 1959 she married Jack Kozak Sr. and had 5 children.
Bev was a devoted homemaker and took great pride in her home. She enjoyedspending time outdoors working on her lawn andtending to her gardens. She also loved volunteering at the Thrift Shop and did so for many years whereshe madelife long friends.
Bev is survived by her children; Joe (Michelle) Kozak, Kim Kozak, Jack Jr. (Nicole) Kozak, Randy (Tammy) Kozak andRick (Tina) Kozak; grandchildren, Roxanne Zamba, John (Stacie) Nichols, Bryan, Brandon, Brittany, Brodi, Nathan and Seth Kozak.
She was preceded in death by her parents, husband Jack Sr. and granddaughter Amanda Rimmer-Kozak.
A memorial service will be held on Tuesday, June 28, 2011 at 5 p.m. at Bethany Ev. Lutheran Church with Pastor Ethan Steinbrenner officiating. Friends may call from 4 p.m. at the church until the time of the service.
Burial will take place at a later date in Lakeview Cemetery.
